RESUMO

Polymers functionalized with inorganic silane groups have been used in wide-ranging applications due to the silane reactivity, which enables formation of covalently-crosslinked polymeric structures. Utilizing stimuli-responsive polymers in these hybrid systems can lead to smart and tunable behavior for sensing, drug delivery, and optical coatings. Previously, the thermoresponsive polymer poly(N-isopropyl acrylamide) (PNIPAM) functionalized with 3-(trimethoxysilyl)propyl methacrylate (TMA) demonstrated unique aqueous self-assembly and optical responses following temperature elevation. Here, we investigate how cosolvent addition, particularly ethanol and N,N-dimethyl formamide (DMF), impacts these transition temperatures, optical clouding, and structure formation in NIPAM/TMA copolymers. Versus purely aqueous systems, these solvent mixtures can introduce additional phase transitions and can alter the two-phase region boundaries based on temperature and solvent composition. Interestingly, TMA incorporation strongly alters phase boundaries in the water-rich regime for DMF-containing systems but not for ethanol-containing systems. Cosolvent species and content also alter the aggregation and assembly of NIPAM/TMA copolymers, but these effects depend on polymer architecture. For example, localizing the TMA towards one chain end in 'blocky' domains leads to formation of uniform micelles with narrow dispersities above the cloud point for certain solvent compositions. In contrast, polydisperse aggregates form in random copolymer and PNIPAM homopolymer solutions - the size of which depends on solvent composition. The resulting optical responses and thermoreversibility also depend strongly on cosolvent content and copolymer architecture. Cosolvent incorporation thus increases the versatility of inorganic-functionalized responsive polymers for diverse applications by providing a simple way to tune the structure size and optical response.

RESUMO

Background: Cardiac diseases are among the leading causes of death worldwide, including sudden cardiac arrest in particular. Nursing professionals are often the first to encounter these scenarios in various settings. Adequate preparation and competent knowledge among nurses significantly impact survival rates positively. Aim: To describe the state of knowledge about Basic and Advanced Life Support guidelines among Ecuadorian nursing professionals. Methodology: A nationwide, descriptive, cross-sectional study was conducted from February to April 2023 among Ecuadorian nursing professionals. Participants were invited through official social media groups such as WhatsApp and Facebook. The study utilized a self-administered online questionnaire to evaluate theoretical knowledge of Basic Life Support (BLS) and Advanced Life Support (ALS). Knowledge scores were assigned based on the number of correct answers on the tests. T-tests and one-way ANOVA were used to examine relationships between knowledge scores and demographic and academic training variables. Results: A total of 217 nursing professionals participated in the study. The majority of the participants were female (77.4%) and held a university degree (79.9%). Among them, only 44.7% claimed to have obtained a BLS training certificate at least once, and 19.4% had ALS certification. The overall BLS knowledge score (4.8/10 ± 1.8 points) was higher than the ALS score (4.3/10 ± 1.8 points). Participants who had obtained BLS certification and those who used evidence-based summaries as a source of extracurricular training achieved higher BLS and ALS knowledge scores. Conclusion: Ecuadorian nursing professionals in this study exhibited a significant deficiency in theoretical knowledge of BLS and ALS. Formal training and preparation positively impact life support knowledge. Support and inclusion of Ecuadorian nurses in training and academic preparation programs beginning at the undergraduate level are essential for promoting life support knowledge and improving outcomes.

RESUMO

Purpose: Cardiorespiratory arrest's unpredictability poses a global health challenge, with gaps in physicians' life support knowledge potentially leading to poor patient outcomes, a factor yet unstudied among Ecuadorian physicians. This study aims to elucidate the state of physicians' theoretical knowledge in Ecuador based on Basic Life Support (BLS) and Advanced Life Support (ALS) guidelines. Patients and methods: A national cross-sectional online 35-questions survey was conducted between February and March 2023 using a self-administered, expert-validated questionnaire. Participants' responses were obtained through official social media groups (WhatsApp and Facebook). The survey evaluated the theoretical knowledge of BLS and ALS, with scores based on the number of correct answers out of a maximum of 10.0 points. For descriptive analysis, frequencies, percentages, means, and standard deviations (SD) were used. The T-test and one-way ANOVA were utilized to analyze the associations between knowledge levels and demographic and academic training variables of Ecuadorian doctors. Values of p < 0.05 were considered statistically significant for all analyses. Results: The survey garnered responses from 385 physicians, with a majority being female (56.6%) and possessing less than 3 years of work experience (75.1%). Of these, 71.7% and 51.9% held BLS and ALS certifications, respectively. Knowledge scores for BLS (5.8/10 ± 1.6) surpassed those for ALS (4.7/10 ± 1.8) (p < 0.001). Physicians with less than 3 years of work experience exhibited higher knowledge scores in both BLS and ALS tests (p < 0.05). Conclusion: This study revealed a notable deficiency in the theoretical knowledge of BLS and ALS among surveyed Ecuadorian physicians. Factors such as prior certification and years of work experience appeared to influence knowledge levels. Continual training and updates in life support protocols at universities and healthcare institutions are key to enhancing physicians' skills and patient outcomes.

RESUMO

BACKGROUND: Microtia is a frequent congenital deformity of the pinna, often with hearing loss. This study reviews the clinical profiles of microtia pediatric patients treated at a referral hospital in Quito, Ecuador, from 2015 to 2022. METHODS: A cross-sectional descriptive study was carried out based on the analysis of medical records of pediatric patients with microtia treated between January 2015 and December 2022 at the Hospital Pediátrico Baca Ortiz in Quito, Ecuador. Descriptive statistics were used, and the Chi-square test assessed associations between categorical variables. RESULTS: Of the 235 patients evaluated, 59.6% were male, 83.4% lived at high altitudes (2500-3500 m), and 19.1% had a family history of microtia. Grade III microtia was diagnosed in 63.8%, predominantly on the right side. Nearly all (99.1%) had hearing loss. Other anatomical alterations were observed in 27.7%, primarily the preauricular appendage. Bone vibrator implantation was a common treatment for 24.3%. Altitude did not show a significant correlation with microtia characteristics. CONCLUSIONS: Most patients had grade III microtia with associated hearing loss. Despite the high prevalence at elevated altitudes, no significant altitude-disease correlation was found. The study highlights the need for further research on microtia in regions like Ecuador.

RESUMO

Much of our current understanding of microbiology is based on the application of genetic engineering procedures. Since their inception (more than 30 years ago), methods based largely on allelic exchange and two-step selection processes have become a cornerstone of contemporary bacterial genetics. While these tools are established for adapted laboratory strains, they have limited applicability in clinical or environmental isolates displaying a large and unknown genetic repertoire that are recalcitrant to genetic modifications. Hence, new tools allowing genetic engineering of intractable bacteria must be developed to gain a comprehensive understanding of them in the context of their biological niche. Herein, we present a method for precise, efficient and rapid engineering of the opportunistic pathogen Pseudomonas aeruginosa. This procedure relies on recombination of short single-stranded DNA facilitated by targeted double-strand DNA breaks mediated by a synthetic Cas9 coupled with the efficient Ssr recombinase. Possible applications include introducing single-nucleotide polymorphisms, short or long deletions, and short DNA insertions using synthetic single-stranded DNA templates, drastically reducing the need of PCR and cloning steps. Our toolkit is encoded on two plasmids, harboring an array of different antibiotic resistance cassettes; hence, this approach can be successfully applied to isolates displaying natural antibiotic resistances. Overall, this toolkit substantially reduces the time required to introduce a range of genetic manipulations to a minimum of five experimental days, and enables a variety of research and biotechnological applications in both laboratory strains and difficult-to-manipulate P. aeruginosa isolates.

RESUMO

The degradability of the biocomposite produced from a binary mixture of thermoplastic banana starch (TPS) and polycaprolactone (PCL) reinforced with fique fibers (Fs) was evaluated in three different environments (soil, compost, water). An experimental design with two factors (soil and compost) and three levels (5, 10, and 20 cm) was used, with additional tests for a third aqueous environment (water from the lake of the Universidad del Valle) at a depth of 20 cm. The biocomposite was prepared from the implementation of a twin-screw extrusion process of the binary mixture TPS/PCL and fique fibers (54, 36, and 10% composition, respectively), followed by hot compression molding, and after that, generating ASTM D638 type V specimens using a stainless-steel die. The specimens were dried and buried according to the experimental design, for a total experimental time of 90 days, and removing samples every 30 days. After 90 days, all samples showed signs of degradation, where the best results were obtained in the compost at a depth of 20 cm (34 ± 4% mass loss and a decrease in tensile strength of 77.3%, which indicates that the material lost mechanical properties). TPS was the fastest disappearing component and promoted the degradation of the composite material as it disappeared. Finally, the aqueous media presented the lowest degradation results, losing only 20% of its initial mass after 90 days of the experiment, being the least effective environment in which the biocomposite can end up.

Aortic dissection represents a relatively rare but highly fatal condition that is due to a tear in the intimal layer ofthe aortic artery or bleeding within its wall, resulting in a separation of its layers. Several risk factors have beenidentifi ed and share the same as for other cardiovascular diseases such as age, high blood pressure, dyslipidemiaand genetic disorders of connective tissue. It is very important that the clinician can make a timely diagnosis and appropriate treatment in patients affectedby it; the type of treatment depends on the extent and location of the dissection. open surgical repair is usuallythe most used for those that involve the ascending aorta and the aortic arch, on the other hand, endovascularintervention is recommended for dissections of the descending aorta that are usually complicated. In this work, the epidemiology, pathophysiology, clinical manifestations and finally the management of aorticdissection will be reviewed.(AU)

The diabetic foot (PD) represents one of the chronic complications of diabetic disease, which occurs due to structur-al and functional alterations of the foot, causing, among others, diabetic foot ulcers (DFU). Mortality rates associatedwith the development of a DFU are estimated at 5 % in the first year, and 42 % at five years. A potentially preventableinitiating event can often be identified, such as minor trauma causing a skin lesion. The classic presentation of DFUis that of a severely infected foot through a previous wound, with rapidly progressive necrosis, necrotic tissue andpurulent collections are observed. Foot amputations, many of which can be prevented with early recognitionand treatment, may be required in up to 20 % of diabetic foot ulcers. Routine screening for neuropathic and vascularinvolvement of the lower extremities and careful inspection of the feet can substantially reduce the morbidity offoot problems. Effective treatment of DFUs is complex and requires considerable expenditure of resources andsignificant cost to the health care system. This review provides a description of the pathophysiology, epidemiology, clinical presentation, diagnosis, and treatment of DFU.(AU)

RESUMO

Inflammatory bowel disease (IBD) is a public health challenge and the use of pectin for symptom amelioration is a promising option. In this work, sunflower pectin has been extracted without (CHP) and with assistance of ultrasound (USP) using sodium citrate as a food-grade extracting agent. At optimal conditions (64 °C, 23 min) the highest yield was obtained with ultrasound application (15.5 vs. 8.1 %). Both pectins were structurally characterized by 1H NMR, HPSEC-ELSD, FT-IR and GC-FID. Unlike CHP, USP showed a lower molecular weight, higher galacturonic acid, lower degree of methyl-esterification and, overall, higher viscosity. These characteristics could affect the anti-inflammatory activity of pectins, evaluated using DSS-induced IBD model mice. So, USP promoted the defence (ICAM-1) and repair of the gastrointestinal mucosa (TFF3, ZO-1) more effectively than CHP. These results demonstrate the potential amelioration of acute colitis in IBD mice through USP supplementation. Taking into account the biomarkers analysed, these results demonstrate, for the first time, the positive impact of sunflower pectin extracted by ultrasound under very soft conditions on inflammatory bowel disease that might open up new possibilities in the treatment of this serious pathology.

Peripheral arterial disease occurs due to obstruction at the arterial level. This obstruction is mainly due to atheroscle-rosis resulting in turn from the accumulation of lipids and fibrous material between the tunica intima and muscularispropria of the blood vessel wall. The consequent decrease in blood flow can present asymptomatically or manifestwith symptoms of arterial insufficiency such as: intermittent claudication, pain at rest in the affected muscle group, tissue loss (ulcers), the latter being one of the most evident signs of ischemia of the the limb. The risk factors aresimilar to cardiovascular risk factors (hypertension, smoking, hyperlipidemia, diabetes or metabolic syndrome). Tocarry out the diagnosis of it, an adequate clinical history is required, an exhaustive physical examination (decreasedperipheral pulses), the ankle-brachial index (ABI) and also the use of diagnostic tools such as: duplex, tomographycomputed and magnetic resonance imaging. Management will depend on the stage of the disease and is aimedat relieving symptoms and reducing the risk of progression of cardiovascular disease.(AU)

RESUMO

The Internet of Things (IoT) paradigm has revolutionized several industries (e.g., manufacturing, health, transport, education, among others) by allowing objects to connect to the Internet and, thus, enabling a variety of novel applications. In this sense, IoT devices have become an essential component of smart cities, allowing many novel and useful services, but, at the same time, bringing numerous cybersecurity threats. The paper at hand proposes BlockSIEM, a blockchain-based and distributed Security Information and Event Management (SIEM) solution framework for the protection of the aforementioned smart city services. The proposed SIEM relies on blockchain technology to securely store and access security events. Such security events are generated by IoT sentinels that are in charge of shielding groups of IoT devices. The IoT sentinels may be deployed in smart city scenarios, such as smart hospitals, smart transport systems, smart airports, among others, ensuring a satisfactory level of protection. The blockchain guarantees the non-repudiation and traceability of the registry of security events due to its features. To demonstrate the feasibility of the proposed approach, our proposal is implemented using Ethereum and validated through different use cases and experiments.

RESUMO

It is well known that one of the main problems in galactooligosaccharide production (GOS) via tranglycosylation of lactose is the presence of monosaccharides that contribute to increasing the glycaemic index, as is the case of glucose. In this work, as well as studying the effect of ultrasound (US) on glucose oxidase (Gox) activation during gluconic acid (GA) production, we have carried out an investigation into the selective oxidation of glucose to gluconic acid in multienzymatic reactions (ß-galactosidase (ß-gal) and Gox) assisted by power US using different sources of lactose as substrate (lactose solution, whey permeate, cheese whey). In terms of the influence of matrix on GOS and GA production, lactose solution gave the best results, followed by cheese whey and whey permeate, salt composition being the most influential factor. The highest yields of GOS production with the lowest glucose concentration and highest GA production were obtained with lactose solution in multienzymatic systems in the presence of ultrasound (30% amplitude) when Gox was added after 1 h of treatment with ß-gal. This work demonstrates the ability of US to enhance efficiently the obtainment of prebiotic mixtures of low glycaemic index.

RESUMO

Intensive exploitation of mahogany wood (Swietenia macrophylla, Meliaceae) has resulted in the loss of natural populations. Somatic embryogenesis offers an alternative to clonal propagation and conservation of mahogany. This study describes biochemical (carbohydrates, total phenols, total flavonoids, protein, and plant growth regulators content) and histological characteristics of the somatic embryogenesis process in mahogany. Calli were obtained by culturing cotyledons of seeds from immature fruits for six weeks on semi-solid MS medium supplemented with 1.0 mgL-1 of kinetin and 4.0 mgL-1 of 2, 4-D. Primary callus was cultured on half strength semi-solid MS medium supplemented with 1.0 mgl-1 6-BA (6-benzylaminopurine) and embryogenic structures were obtained. Embryo development from globular-shaped somatic embryos to the cotyledonary stage was confirmed by histology and scanning electron microscopy. Shoot initiation was observed after somatic embryos were transferred to germination and maturation medium. Endogenous concentrations of carbohydrates, total phenols, total flavonoids, protein, and plant growth regulators were determined in embryogenic (EC) and non-embryogenic (NEC) calli of mahogany. Embryogenic cultures contained significantly higher concentrations of IAA (indoleacetic acid), ABA (abscisic acid), and GAs (Gibberellins 1+3+20), whereas non-embryogenic calli contained more total phenols, flavonoids and resistant starch. Fructose and glucose were not present at detectable levels in EC or NEC, whereas soluble starch and sucrose were only detectable in EC. Concentrations of total proteins, Z/ZR (Zeatin/zeatin riboside) and iP/iPA (N6-(Δ2-isopentenyl) adenine and N6-(Δ2-isopentenyl) adenosine) were similar in EC and NEC.

An optimization of temperature, time, and extracting agent concentration of pectin extraction from sunflower heads using sodium citrate and nitric acid (SP-SC and SP-NA) was carried out. At optimal conditions, the yield of extraction with nitric acid (SPO-NA) was twofold greater than the corresponding with sodium citrate (SPO-SC) (14.3 versus 7.7%, respectively). Regarding pectin structure, the galacturonic acid (GalA) content in both, SPO-SC and SPO-NA, was similar (∼85%). However, SPO-NA showed lower molecular weight (Mw) (88.9 kDa) and neutral sugar content (4%) than SPO-SC (464 kDa, 9%), indicating that nitric acid deeply degraded pectin structure. These differences derived into dissimilar behavior in their technological functionality. SPO-SC showed higher viscosity and better emulsifying capacity than SPO-NA, although any of them were able to stabilize the oil/water emulsion. Both sunflower pectins formed gels with Ca2+ (75 mg/g of pectin) at pH 3.0. However, when sucrose was added, the gels formed by SP-SC and 20% sucrose presented the same hardness as those of SP-NA with 40% sucrose. These results suggest that the pectin extracted with sodium citrate, an eco-friendly agent, could be a promising ingredient, with good thickening and gelling properties.

RESUMO

Galactooligosaccharides (GOS), recognised prebiotic, can be industrially produced from lactose and commercial ß-galactosidase (ß-gal) from Kluyveromyces lactis. Residual lactose and glucose limit GOS applications. To handle this problem, a multienzymatic system, with ß-gal and glucose oxidase (Gox), was proposed to reduce glucose content in reaction media through its oxidation to gluconic acid (GA). Besides, ultrasound (US) probe effect over the multienzymatic system to produce GOS and GA has been evaluated. A production around 40% of GOS was found in all treatments after the first hour of reaction. However, glucose consumption and GA production was significantly higher (P < 0.05) for sequential reaction assisted by US, obtaining the best production of GOS (49%) and GA (28%) after 2 h of reaction. The conformational and residual activity changes of enzymes under US conditions were also evaluated, Gox being positively affected whereas in ß-gal hardly any change was found.

RESUMO

A high-performance size-exclusion chromatography (HPSEC) method coupled to Evaporative Light Scattering (ELS) and Refractive Index (RI) detectors were evaluated and compared for the molecular mass (Mw) estimation of pectin in a wide range (0.342-805 kDa). Instrumental parameters of the ELSD were optimised by Response Surface Methodology (RSM) being 73 °C the evaporator temperature and 0.9 mL/min the air flow rate. The linear range for the ELSD concentration response was wider (10-2250 mg/L) and better (R2 = 0.985) than RID (10-1500 mg/L; R2 = 0.875). The limits of detection (LOD) and quantitation (LOQ) for all pullulans hardly changed in ELSD (LOD: 1.22-1.99 mg/L; LOQ: 4.07-6.63 mg/L); however, RID showed huge variations (LOD: 0.49-10.41 mg/L; LOQ: 1.64-34.70 mg/L), which increased with the Mw. In general, responses of both detectors were similar for the Mw estimation, although pectin characterisation with HPSEC-ELSD exhibited better results in the lowest Mw compounds.

RESUMO

BACKGROUND: Corticosteroid infiltration (CI) is commonly used for treatment of plantar fasciosis. In recent years, however, interest has grown in the use of intratissue percutaneous electrolysis (EPI) for the treatment of tendinopathies. The aim of our study was to compare the effectiveness of the above techniques in the treatment of plantar fasciosis. METHODS: The results achieved over a period of 1 year following the use of these techniques to treat plantar fasciosis were examined. There were 64 patients; 32 of whom were treated with ultrasound-guided EPI and 32 with ultrasound-guided CI. A clinical examination was performed and ultrasound taken before treatment and at 3, 6, and 12 months. Clinical assessments were made using a visual analog scale (VAS) to record pain and the Foot and Ankle Disability Index (FADI) to evaluate function. Ultrasound was used to determine the thickness of the plantar fascia. RESULTS: Both the ultrasound-guided EPI and CI techniques were associated with significant clinical and echographic improvements at 12 months post-treatment ( P < .001). CONCLUSION: Both techniques were effective in the treatment of PF, providing excellent VAS pain and FADI results at 12 months. However, CI required fewer patient visits and appeared to provide somewhat better VAS and FADI results. LEVEL OF EVIDENCE: Level III, retrospective comparative study.

ABSTRACT Nowadays the Meta Department has a high hydrological potential, which results in an opportunity for agriculture and livestock. Moreover, activities of the oil industry have significantly increased, which have brought new challenges and threats to the region. Consequently, different types of discharges and environment modifications affect the Meta Region's rivers in their watersheds and their riparian and benthic communities. However, these systems are poorly understood, including the periphyton and macroinvertebrate communities that inhabit them. In this paper, we provide a review of the most relevant characteristics of these communities, and we discuss the changes caused by human activities on the ecological stability of the rivers in a regional context. Additionally, we presented a list of periphytic algae and benthic macroinvertebrates taxa registered in the Meta Region.

RESUMO

Macrophages (Mϕs) of patients with Alzheimer's disease and mild cognitive impairment (MCI) are defective in amyloid-ß1-42 (Aß) phagocytosis and have low resistance to apoptosis by Aß. Omega-3 fatty acids (ω-3s) in vitro and in vivo and the ω-3 mediator, resolvin D1, in vitro increase Aß phagocytosis by Mϕs of patients with MCI. We have investigated the unfolded protein response (UPR) to endoplasmic reticulum (ER) stress by Mϕs in a longitudinal study of fish-derived, ω-3-supplemented patients with MCI. Patients in the apolipoprotein E (ApoE)e3/e3 subgroup over time exhibited an increase of protein kinase RNA-like ER kinase (PERK) expression, Aß phagocytosis, intermediate M1-M2 Mϕ type, and a Mini-Mental State Examination (MMSE) rate of change of +1.8 points per year, whereas patients in the ApoEe3/e4 subgroup showed individually divergent results with an MMSE rate of change of -3.2 points per year. In vitro treatment of Mϕs by fish-derived ω-3 emulsion increased Aß phagocytosis, PERK expression, and UPR RNA signature, and decreased ER stress signature. Augmented genes in the UPR signature included chaperones, lectins, foldases, and N-linked glycosylation enzymes. In summary, fish-derived ω-3s increase cytoprotective genes and decrease proapoptotic genes, improve immune clearance of Aß, and are associated with an improved MMSE rate of change in ApoEe3/e3 vs. ApoEe3/e4 patients.-Olivera-Perez, H. Omega-3 fatty acids increase the unfolded protein response and improve amyloid-ß phagocytosis by macrophages of patients with mild cognitive impairment.

RESUMO

Galanin (GAL) is a neuropeptide involved in the homeostasis of energy metabolism. The objective of this study was to investigate the serum levels of GAL during an oral glucose tolerance test (OGTT) in lean and obese young men. This cross-sectional study included 30 obese non-diabetic young men (median 22 years; mean BMI 37 kg/m(2)) and 30 healthy lean men (median 23 years; mean BMI 22 kg/m(2)). Serum GAL was determined during OGTT. The results of this study include that serum GAL levels showed a reduction during OGTT compared with basal levels in the lean subjects group. Conversely, serum GAL levels increased significantly during OGTT in obese subjects. Serum GAL levels were also higher in obese non-diabetic men compared with lean subjects during fasting and in every period of the OGTT (p < 0.001). Serum GAL levels were positively correlated with BMI, total fat, visceral fat, HOMA-IR, total cholesterol, triglycerides and Leptin. A multiple regression analysis revealed that serum insulin levels at 30, 60 and 120 minutes during the OGTT is the most predictive variable for serum GAL levels (p < 0.001). In conclusion, serum GAL levels are significantly higher in the obese group compared with lean subjects during an OGTT.
